How is lorazepam usually taken?

This page covers the US label for lorazepam tablets taken by mouth, with the dosing the label gives for adults.

This is label-level information, not personal advice. Indian pack labels may differ, and your prescriber's instruction always comes first.

The label describes a usual range of 2 to 6 mg a day in divided doses, with the largest dose at bedtime. The daily dose may range from 1 to 10 mg. For anxiety, most patients need an initial 2 to 3 mg a day, given two or three times a day.

For insomnia due to anxiety or temporary stress, a single dose of 2 to 4 mg at bedtime may be given. For elderly or debilitated patients, it describes an initial 1 to 2 mg a day in divided doses.

Doses should be raised gradually. To stop it, the label says to use a gradual taper to reduce withdrawal reactions.

  • Usual range 2 to 6 mg a day in divided doses
  • Elderly or debilitated: start 1 to 2 mg a day
  • Increase gradually
  • Taper slowly when stopping

Who should be careful with lorazepam?

The label warns that using benzodiazepines with opioids can cause profound sedation, slowed breathing, coma, and death. It also warns of abuse, misuse, addiction, dependence, and withdrawal reactions, so a gradual taper is needed when stopping.

It must not be used by people allergic to benzodiazepines or with acute narrow-angle glaucoma. Elderly or debilitated patients need lower doses.

Neonates born to mothers using benzodiazepines late in pregnancy may have sedation or withdrawal symptoms. Do not drive until the effects of use with an opioid are known.

  • Serious risk when combined with opioids
  • Risk of abuse, addiction, dependence and withdrawal
  • Do not use if allergic to benzodiazepines or with narrow-angle glaucoma
  • Lower dose in elderly
  • Late pregnancy use may cause newborn sedation or withdrawal
