How does torsemide work?
Torsemide acts in the thick ascending loop of Henle in the kidney, where it blocks the Na+/K+/2Cl- carrier.
This increases urinary excretion of sodium and water.
It makes the kidneys pass more salt and water in urine.

How is torsemide usually taken?
This page covers the US label for torsemide tablets taken by mouth, with the dosing the label gives for adults.
This is label-level information, not personal advice. Indian pack labels may differ, and your prescriber's instruction always comes first.
For edema with heart failure, the label describes an initial dose of 10 or 20 mg once daily, and 20 mg once daily with chronic kidney failure. In cirrhosis, it describes 5 or 10 mg once daily with an aldosterone antagonist or potassium-sparing diuretic.
For hypertension, it describes 5 mg once daily, which may rise to 10 mg after 4 to 6 weeks.
Doses may be raised by roughly doubling. Doses above 200 mg (above 40 mg in cirrhosis) have not been adequately studied.
- Heart failure edema: 10 or 20 mg once daily
- Kidney failure edema: 20 mg once daily
- Cirrhosis edema: 5 or 10 mg once daily
- Hypertension: 5 mg once daily
The label describes a once-daily dose that depends on the condition and is adjusted to response.

Who should be careful with torsemide?
The label says it is contraindicated in hypersensitivity to torsemide or povidone, in anuria, and in hepatic coma.
Excess diuresis can cause dehydration, low blood pressure and worsening kidney function, especially in salt-depleted patients. Volume status, kidney function and electrolytes need periodic monitoring.
Ototoxicity risk is higher with high doses, severe kidney impairment and low blood protein. Gout may rarely be triggered.
- Not in anuria or hepatic coma
- Not if allergic to torsemide or povidone
- Dehydration and low blood pressure risk
- Electrolytes and kidney function need monitoring
- Hearing effects at high doses
It can cause dehydration and electrolyte loss, so regular monitoring is needed.
